Integrated Care Approaches for Managing Parkinson’s Disease and Comorbidities in Elderly Patients

Abstract

This case report describes the clinical course, multidisciplinary management and outcomes of a 60-year-old patient with advanced Parkinson’s disease (PD) presenting with comorbidities and the akinetic-rigid form of tremor. The report highlights the effectiveness of an integrated rehabilitation approach to improve motor function and quality of life (1).

The study focused on the case of a 60-year-old woman diagnosed with stage 3 PD in the form of akinetic-rigid-tremor, which presented with recurrent tremors, altered gait, dyskinesia and muscle stiffness (5). Treatment included pharmacological interventions such as levocarbisan, pramipexole, rasagiline and amitriptyline, as well as a structured rehabilitation program including physiotherapy, mechanotherapy, cognitive exercises, tai
chi, voice therapy and breathing exercises.(6). Motor function and quality of life were assessed using the Barthel index and MMSE scores before and after the intervention (7).

After rehabilitation, the patient showed a significant improvement in motor function, as evidenced by an increase in Barthel scale score from 70 to 85 and a decrease in tremor intensity (8). Gait and balance also improved and cognitive function remained stable (MMSE 28) (9). These results emphasize the importance of a multidisciplinary approach in the management of advanced PD with comorbidities and highlight the role of rehabilitation to address complex clinical needs, in accordance with WHO recommended practices (10).

This case illustrates the challenges of managing advanced PD in comorbid patients and highlights the potential benefits of a comprehensive multidisciplinary rehabilitation program. Motor function and quality of life support the need for individualized care plans that conform to established best practices for PD management (11).
